Your personal information, including your personal health information, is entirely confidential.

Your employer, insurance provider, or family members do not have access to any of the information. This includes primary account holders and their family members over the age of 14.

Children under the age of 14 do not hold personal accounts, thus the parent who is the main account holder has the ability to consult them.

All health care providers and other providers engaged by Dialogue, whether as contractors or employees, are contractually obligated to keep personal information confidential, use it only for the purposes of providing the services you have requested and ensure that when processing personal information that it is in compliance with applicable privacy Laws.

We will not disclose your personal information, including personal health information, for any purpose other than what has been outlined in our Privacy Policy or as permitted or required under applicable law, unless we obtain your consent. We disclose only the limited amount of personal information including personal health information necessary to meet these purposes.
