For medications prescribed outside of Dialogue
When you are renewing a medication, our intake process will require you to upload a photo of the label of your previous prescription. If you do not have a copy of this label, please reach out to your pharmacy and ask them to send us a copy via fax. Our fax number is (514) 316-6625.
Please make sure to save the chat you had started by clicking on the X at the top, then SAVE. This will allow our care team to notify you once we have received and filed your prescription.
Finally, if no copy of the prescription you are trying to renew is available, please consult for a new health issue instead of a prescription renewal. Please note we cannot guarantee that this medication will be prescribed without a valid proof of previous prescription.
For medications prescribed by Dialogue practitioners
If this medication was most recently prescribed by a Dialogue practitioner, we will have the necessary documentation on file.
Note: For some medications, you might also be asked to provide other information or documentation, such as proof of recent blood pressure readings or to fill out additional questionnaires before an appointment time can be offered.
