To change your name, home address or date of birth in your account, or that of a family member 13 years old and younger, please log into your account and continue with scheduling an appointment, whenever you are ready to do so. Once scheduled, you can reach out in the chat where the appointment is confirmed to ask our care team to modify the information in your account. They will be able to make these changes for you.
Please note this information will not prevent any appointments from being scheduled. It would simply be important that this be fixed before any prescriptions or documents are drafted by your practitioners.
If you are trying to edit the name, address or date of birth of a family member 14 years old and over, please ask them to make the request through their own account, following the same steps as above.
